Counseling visits in the home environment for people in need of care of all ages and family caregivers.
Professional advice and support for family members providing care
Organization of support measures, aids and establishing contacts with care providers

Training measures on violence in care
Creation and implementation of an individual care & assistance plan
Support with bureaucratic issues relating to care degree applications, objection procedures, rehabilitation and integration measures, disability degree applications, living wills and health care proxies.

This is how everything proceeds
Care consultation for those in need of care and the process involved are standardized throughout Germany in Section 37.3 of the 11th Social Security Code. I would also like to offer you the highest level of transparency and answer all of your important questions so that those in need of care and their relatives receive the best possible support.

Clarification of care needs:
At the beginning, the individual care needs of the person affected are determined. This includes the health situation, the need for support in everyday life, the housing and living situation, mobility and the situation of the caring relatives. Here it is determined exactly what kind of help is necessary. -
Creation of the individual care plan:
Based on the information collected, a tailor-made care plan is developed. This plan specifies what care and support the person in need of care should receive and how this can best be organised. -
Provision of services and aids:
In this step, the necessary services and aids are organized. This can include arranging care services, providing care aids or organizing medical and social services. -
Support in implementing the care plan:
Once the plan has been drawn up, support is provided with practical implementation. This means that the agreed actions are coordinated and monitored to ensure that everything goes as planned. -
Support and adaptation:
The person in need of care and their relatives will continue to receive support to ensure that the care plan continues to work in the long term. If the person in need of care's condition changes, the plan will be adjusted. -
Services to relieve the burden on the caregiver:
Caring relatives are relieved of their burden through various measures, such as short-term care, day and night care or the provision of nursing aids. This ensures rest and support for the caring person. -
Termination of care consultation:
The process ends when all measures are established and nursing care is stable. However, the nursing consultation process can be resumed at any time if needs change.
